Checklist 12A – Registration of Foreign Decree Under Uniform Child Custody Jurisdiction and Enforcement Act (UCCJEA) – Word

Once the forms are completed, you will need to make 2 copies (original + 2 copies = 3 total) of each required form. All forms must be reviewed by our Compliance Officer before they can be filed.
