Checklist 5E – With Children – Word

• If filing as a Self-represented Party (without the aid of an attorney) you must print out Checklist 5E along with the required forms. Once the forms are completed, you will need to make 2 copies (original + 2 copies = 3 total) of each required form. All forms must be reviewed by our Compliance Officer before they can be filed.
• Forms can be accessed by first clicking on the desired format, Word or PDF, and secondly by clicking on the appropriate form number.

Without Children

Checklist 5F – No Children – Word
